Download Record Slips



Records are the life-blood of the DOS. By sending in your records you can directly help us in the conservation of birds and their habitats.

The records will be used to complie the annual Derbyshire Bird Report and the monthly (except July) Bird Notes section of the DOS Bulletin. They will ultimately end up on the DOS Records Database which forms a unique resource of bird records for the whole county going back to 1 January 2000.

The link below will allow you to download a sheet of four slips that you can then print off as required.

When completed, slips should be mailed to Bryan Barnacle, Mays, Malthouse Lane, Froggatt, Hope Valley, S32 3ZA
